For physicians · Remote Patient Monitoring
Better blood pressure control between visits.
A Florida pharmacist monitors your hypertensive patients every day, calls them every month, and works within the protocol you approve. Your practice bills the RPM codes and keeps the reimbursement.
The clinical case
An extension of your care
Office readings capture one moment. Remote monitoring captures every day in between, so you and your patient see a clearer picture. Published outcomes show physician-supervised RPM with pharmacist support outperforms office-based care alone:
- Greater BP reduction: physician and pharmacist teams achieve an additional 7–10 mmHg systolic improvement over usual care across multiple meta-analyses.
- Faster time to goal: patients reach target in 6–8 weeks instead of 6+ months of office-based titration.
- Better adherence: frequent contact and pharmacist coaching address non-adherence, the leading reversible cause of treatment failure.
- Earlier intervention: daily data flags hypertensive urgency, white-coat effect and masked hypertension.
- Stronger MIPS quality scores: consistent monitoring improves the Controlling High Blood Pressure measure that feeds the Quality category.
How the partnership works
Four steps, then it runs every month
- You refer the patientAny patient with documented hypertension (ICD-10 I10–I15) who would benefit from closer monitoring. No smartphone needed.
- We enroll and equipThe patient gets a validated cellular blood pressure cuff and setup training, in person or by video (CPT 99453).
- We monitor and manageThe pharmacist reviews every reading, calls the patient at least monthly, and acts within the parameters in your CPA.
- You bill, we documentEach month you receive a clinical summary and the documentation for every eligible CPT code. Your practice bills and keeps 100%.
Lighter load for your office
Fewer calls, fewer refill messages
Under the CPA, the pharmacist handles the steady stream of hypertension questions so your staff can focus on the patients in front of them.
- BP-related patient calls: triaged, answered and documented by the pharmacist.
- Routine refills: managed within CPA parameters.
- Side effects, adherence and lifestyle questions: handled directly.
- Out-of-range readings: we contact the patient first, follow your protocol, and send you a documented summary.
- Monthly clinical summaries: delivered to your EMR or as a secure PDF.

Compliance and coverage
Built to keep your practice protected
- Anti-Kickback and Stark: our service agreement is structured to fit the federal personal services safe harbor and Stark exception.
- You keep oversight: you define the protocol, approve each patient and review monthly summaries. The CPA is revocable in writing at any time.
- Our own liability coverage: Vaccine Van Rx carries pharmacist professional liability. Your malpractice coverage is unaffected.
- Florida scope: pharmacist collaborative practice under F.S. 465.1865 and Rule 64B16-31.007, with Board collaborative practice certification on file.
- HIPAA: a Business Associate Agreement covers every patient record we touch.
Patient cost: Medicare Part B covers RPM at 80%. With a Medigap supplement or Medicaid, the patient typically pays nothing. For Medicare Advantage and commercial plans, we verify benefits before enrollment and tell the patient the exact amount first.
