Vaccine Van Rx — Privacy Policy

Who we are

Vaccine Van Rx is a service brand of Vaccine Van Rx, LLC, a Florida limited liability company owned by Jeffrey Godin, PharmD, a Florida-licensed pharmacist. This policy explains what we collect when you request a clinic or an immunization appointment through vaccinevanrx.com, what we do with it, and who receives it.

Vaccine Van Rx, LLC is not the dispensing pharmacy for the services you book through this site. Immunizations are administered by Jeffrey Godin, PharmD, Florida license PS49125, under the standing order protocol of My Concierge Pharmacy, PLLC and its supervising physician. Prescription products are dispensed by My Concierge Pharmacy, PLLC, a Florida-permitted community pharmacy (permit #PH 34867), which acts as the dispensing pharmacy of record and submits your insurance claim.

Two sets of rules protect your information. What you give Vaccine Van Rx, LLC through this site is governed by this policy and by federal and Florida consumer privacy law. Once it reaches My Concierge Pharmacy, PLLC, it becomes part of a pharmacy record governed by HIPAA and by that pharmacy's Notice of Privacy Practices. Jeffrey Godin, PharmD, is bound by HIPAA and by Florida pharmacy law in his practice as a pharmacist.

What we collect

We collect only what is needed to schedule your appointment and let the dispensing pharmacy fill and bill for it.

How we use it

We use your information to:

• Confirm you are eligible for the vaccines you requested
• Schedule your appointment or clinic and send you confirmations and reminders
• Pass your insurance details to My Concierge Pharmacy, PLLC, which verifies your benefits and bills your insurer
• Transmit your information to My Concierge Pharmacy, PLLC so it can dispense your vaccine and report your immunization to the Florida SHOTS registry
• Prepare for your visit, including the pharmacist's clinical review of allergies, conditions and prior doses
• Answer your questions and respond to requests
• Meet our legal, insurance and recordkeeping obligations

We do not use your health information to make decisions about credit, employment, housing or insurance eligibility, and we do not provide it to anyone who does.

Who we share it with

My Concierge Pharmacy, PLLC. Everything needed to dispense and bill for your vaccine goes to the dispensing pharmacy of record. This is the central disclosure this policy exists to explain, and you authorize it when you submit a request.

The administering pharmacist. Jeffrey Godin, PharmD, reviews your information before and during your visit.

Service providers. Scheduling, hosting, email and payment vendors that process information on our behalf, under contract, and only for the purposes above.

When the law requires it. Public health reporting, a subpoena or court order, a regulator with jurisdiction, or to prevent serious harm.

With your permission. A caregiver, family member, employer clinic sponsor or facility you tell us to include.

We do not sell your personal information, and we do not share it with advertisers or data brokers.

What happens once the pharmacy has it

Once your information reaches My Concierge Pharmacy, PLLC, that pharmacy holds it as a HIPAA covered entity. From that point:

• Your prescription and dispensing records are pharmacy records, created and kept by My Concierge Pharmacy as Florida law requires of a permit holder
• Your immunization is reported to Florida SHOTS under that pharmacy's registry account
• Your insurance claim is submitted by that pharmacy
• Your rights in those records (access, amendment, an accounting of disclosures, restrictions) come from HIPAA and are exercised with that pharmacy, under its Notice of Privacy Practices

Vaccine Van Rx, LLC keeps its own copy of the information you gave us, governed by this policy. The two sets of records are separate, and this policy does not control what the pharmacy does with its records.

Communications from us

We send two kinds of messages.

Service messages relate to a visit you requested: confirmations, reminders, changes, follow-up instructions, and answers to your questions. You receive these as part of the service and cannot opt out of them while an appointment is active.

Service and clinic updates tell you about vaccines coming into season, clinics near you, new services, and reminders that a dose may be due. We send these only if you opt in, and you can opt out at any time using the unsubscribe link in any email, by replying STOP to a text message, or by contacting us at the address below. Opting out does not affect your care or your ability to book.

We do not sell your information to advertisers, and we do not send you marketing on behalf of anyone else.

How long we keep it, and how we protect it

We keep appointment and health information for five years after your last visit, and contact information until you ask us to delete it or opt out. Insurance details are deleted once the pharmacy has processed the claim. Site analytics are kept for 12 months.

We protect your information with encryption in transit and at rest, access limited to the pharmacist and staff who need it, multi-factor authentication on systems that hold health information, written agreements with our service providers, and cyber liability insurance.

No system is perfectly secure. If something goes wrong, the next section says what we will do.

Your choices

You can ask us to:

• Send you a copy of the information we hold about you
• Correct anything that is wrong or out of date
• Delete your information, except what we must keep for legal, insurance or recordkeeping reasons
• Stop contacting you about clinics and services
• Stop collecting site analytics, by using your browser's do-not-track setting or declining non-essential cookies

Write to us at the address at the end of this policy. We will confirm within 10 business days and respond within 30 days. We may need to verify who you are before we act.

These choices apply to the records Vaccine Van Rx, LLC holds. For records held by My Concierge Pharmacy, PLLC, contact that pharmacy directly. Your HIPAA rights there are broader than what this policy provides, and deleting our copy does not delete theirs.

If there is a breach

If your personal information is exposed in a security breach, we will notify you as the Florida Information Protection Act requires, no later than 30 days after we determine a breach occurred, unless a law enforcement agency asks us in writing to wait. Where the law requires it, we will also notify the Florida Department of Legal Affairs and the credit reporting agencies.

Our notice will tell you what happened, when, what information was involved, and what we are doing about it.

Patients under 18

A parent or legal guardian must book the appointment, provide the information, and give consent for any patient under 18. We do not knowingly accept bookings made by minors on their own behalf.

A minor's information is handled the same way as everyone else's under this policy. Marketing messages are sent to the parent or guardian, never to the minor. A parent or guardian may exercise every choice in the section above on the minor's behalf.

If you believe a minor has given us information without a parent or guardian involved, contact us and we will delete it.

Changes and how to reach us

If we change this policy we will post the new version here with a new effective date. If the change materially affects how we use information we already hold, we will email you before it takes effect.
