On-Site Pharmacy Oversight
for Florida Institutions

Our Florida-licensed Consultant Pharmacist provides monthly on-site clinical oversight for assisted living facilities, group homes, and other residential care institutions — replacing the phone-it-in chain pharmacy consultant with engaged, local clinical expertise.

For Facility Administrators

Real Clinical Partnership, Not a Distant Vendor

F.A.C. 58A-5.0185 requires consultant pharmacist oversight for medication management. Most facilities default to whatever consultant their dispensing pharmacy provides — often with minimal actual engagement. We deliver structured monthly service with measurable value.

  • Monthly on-site Medication Regimen Review (MRR)
  • Quarterly staff training on medication safety and administration
  • Annual P&P review and pre-survey readiness assessment
  • As-needed clinical consultations and incident review
  • Available for AHCA inspection support
For Directors of Nursing

A Pharmacist Who Knows Your Residents

When your consultant pharmacist actually visits the facility monthly, reviews charts in person, and is available for clinical questions, your nursing staff gains a true clinical partner — and your residents get better medication care.

  • Polypharmacy review and deprescribing recommendations
  • Drug interaction analysis and dose optimization
  • Geriatric pharmacology expertise (Beers Criteria, STOPP/START)
  • Direct line of communication for urgent clinical questions
  • Bilingual service available (English / Spanish)

Remote Patient Monitoring &
Clinical Management

Our pharmacist-led RPM program brings continuous chronic disease monitoring to patients in their own homes — with daily blood pressure tracking, monthly clinical management, and direct collaboration with your physician.

For Patients

Better Care Between Visits

If you have hypertension or another chronic condition, our pharmacist works with your doctor to monitor your health between office visits — catching problems early and helping your medications work better.

  • Free cellular blood pressure cuff (no setup needed)
  • Daily readings reviewed by your pharmacist
  • Monthly clinical check-ins by phone or video
  • Medication questions answered same-day
  • Most Medicare and commercial insurance covered
For Physicians

Extend Your Practice Without Adding Staff

RPM under a collaborative practice agreement allows our PharmD to manage routine chronic disease monitoring — freeing your team to focus on complex cases while improving outcomes and adding a CMS RPM revenue stream to your practice.

  • Full billing documentation for 2026 CMS RPM codes (99453, 99454/99445, 99457, 99458)
  • General supervision — no co-location required
  • Custom CPA tailored to your practice protocols
  • Monthly outcomes reports back to your office
  • Patient-friendly, HIPAA-compliant communication

Building Florida's First Multi-Physician Mobile Pharmacy Network

Vaccine Van Rx partners with primary care physicians, internists, and cardiologists to deliver scalable RPM services that improve patient outcomes and add a new revenue stream to your practice. We handle the clinical workflow, the devices, and the patient communication; we provide the documentation and billing codes for your practice to bill CMS — you maintain physician oversight and capture the RPM revenue.

Meet Jeffrey Godin, PharmD

Vaccine Van Rx was founded by a clinical pharmacist who has spent his career delivering care across the full breadth of pharmacy practice — and saw a gap worth filling.

Pharmacist-Led, Patient-Centered, Outcomes-Driven

Born and raised in South Florida

Jeffrey Godin, PharmD is a South Florida-born and South Florida-raised clinical pharmacist who has built a career across the full breadth of pharmacy practice. After earning his Doctor of Pharmacy from Palm Beach Atlantic University's Lloyd L. Gregory School of Pharmacy in 2012, Jeff has spent fifteen years managing community pharmacy operations at Publix Pharmacy, ten years practicing emergency medicine pharmacy at Memorial Regional Hospital, and five years providing outpatient oncology specialty pharmacy care at Jupiter Medical Center. He is certified to administer immunizations, holds a Florida Consultant Pharmacist license, and is sitting for the Board Certification in Ambulatory Care (BCACP) in 2026. Across every setting, one thread held constant: patients do best when their pharmacist is accessible, communicative, and genuinely invested in their care.

That conviction shaped Vaccine Van Rx. After years across community, hospital emergency, and oncology outpatient settings, Jeff saw the same gap in each: patients between visits, between systems, and between providers — without a clinical resource bridging them. Mobile immunization, consultant pharmacist services for assisted living facilities, and pharmacist-led remote patient monitoring under collaborative practice agreements address that gap directly. The model brings effective clinical care closer to patients, reduces workload on partner physician practices, and reaches into underserved communities that traditional retail pharmacy too often passes over. In practice, this means a vaccine administered at the kitchen table of a homebound senior, a monthly medication review that catches a duplicate prescription before it causes a fall, or a hypertension trend flagged and titrated within parameters the physician already approved — care delivered where and when patients actually need it.

"Quality clinical care should not require a patient to leave their home, work site, or assisted living facility."

Outside of the practice, Jeff is married to his wife Maria, co-founder of Vaccine Van Rx and his partner in every sense, and is the proud father of two young children, Ace and Capri. The family speaks both English and Spanish at home, and bilingual clinical support is a core part of how VVRx serves its patients and their loved ones. Jeff is customer-service oriented at heart and believes that the foundation of effective clinical care is a strong, trusting relationship — between pharmacist and patient, between pharmacist and physician, and between pharmacist and the broader care team. Whether speaking with an assisted living administrator about resident medication safety, a primary care physician about a collaborative practice agreement, or a patient about their blood pressure trends, Jeff approaches every interaction with the same posture: listen first, follow through, and earn trust through results.

Frequently Asked

How does mobile vaccine administration work?

We bring vaccines to your location — ALF, workplace, or home — in validated cold-chain transport. A licensed pharmacist conducts pre-vaccination screening, obtains informed consent, administers the vaccine, and observes you for 15 minutes afterward. Records are reported to Florida SHOTS automatically.

What insurance do you accept?

We accept Medicare Part B, most Medicare Advantage plans, and major commercial insurance through our Pharmacy Services Administrative Organization (PSAO). Coverage details depend on your specific plan; contact us with your insurance information for verification.

Who can receive vaccines?

All vaccines are administered to adults age 18 and older. Influenza vaccines may also be administered to patients age 7 and older, consistent with Florida Statute §465.189. Pre-vaccination screening identifies any contraindications or precautions.

How does the RPM program work for patients?

Eligible patients receive a cellular blood pressure cuff at no cost. The cuff transmits readings automatically to your pharmacist, who reviews trends and conducts a monthly check-in by phone. The pharmacist works with your physician under a formal collaborative practice agreement to optimize your care.

What conditions does RPM monitor?

Currently, our RPM program focuses on hypertension management. Future service expansions will include diabetes, heart failure, and other chronic conditions — each governed by a specific collaborative practice agreement with your physician.

How do you protect patient privacy and data?

Vaccine Van Rx is fully HIPAA-compliant. All patient data is stored in encrypted, access-controlled systems. Communication with patients and physicians uses secure channels. Patient records are shared only with explicit authorization.

For physicians: how does the collaborative practice agreement work?

Under Florida Statute §465.1865, a Florida-licensed physician may enter into a written collaborative practice agreement with a Florida-licensed pharmacist to delegate specific clinical functions. The CPA defines the scope, formulary, consultation thresholds, and reporting requirements. Each physician partner signs a CPA tailored to their practice.

What's the cost for physicians who partner on RPM?

Our pharmacist delivers the RPM clinical service under your collaborative practice agreement. We provide the billing codes (CPT 99453, 99454/99445, 99457, 99458) along with full documentation and time logs each month, which your practice uses to bill CMS under your NPI. Your practice retains the RPM revenue; we are compensated for the clinical work per the CPA terms. No devices to purchase, no staff to hire, no billing infrastructure to build.
